REGISTRATION AND PAYMENT ARE REQUIRED IN ADVANCE


Payment can be made by credit card, check or money order, or UW Budget Number. Payments for both the UWEB21 Short Course and the NESAC/BIO Workshop must be made in advance.

Credit Card:   Credit Card payments are made online via PayPal. After the registration form is completed you will be re-directed to PayPal's website to securely enter your credit card information.
 
UW Budget:   UW Budget payments are made online on the registration form.

Early Fees apply for registration before and on Monday, July 15th, 2013.

Optional Reference Textbook:
Biomaterials Science: An Introduction to Materials in Medicine, 3rd ed. is available to all paying Short Course attendees for an additional $80. Just released last Fall, this best-selling title provides the most comprehensive review of all aspects of biomaterials science, with over 90 contributors from academia, government, and industry. The book explains the clinical uses of biomaterials as components in implants, devices, and artificial organs. Chapters detail biomaterials uses in biotechnology as well as the characterization of the physical, chemical, biochemical and surface properties of these materials. Also included are regulatory and ethical issues, future directions of the field, and an update on medical and biotechnological applications.

Cancellation Policy
If you are not able to attend the UWEB21 Short Course, you may send another person in your place or request a refund. Refunds will be granted minus a cancellation fee. For cancellations requested through July 15, 2013, the cancellation fee is $50. For cancellations requested July 16th and later, the cancellation fee is $75.
